Apply for scholarship - Prime Time After School - Steiner Branch Prime Time

To apply for the scholarship for Prime Time After School Program, you must complete the Financial Aid Form and provide proof of assistance or proof of income. If your child receives at least one of the benefits or services listed in Section 2, they will be approved. You must provide proof of everything that you check ‘yes’ for, but you only have to check ‘yes’ for one thing in Section 2 to be approved. You will have to upload a screenshot of proof from Georgia Gateway (instructions here). The proof will be two pages, one that has the child’s name and the other that has the coverage dates. If you do this, you may skip Sections 3 and 4 and proceed to Section 5.

If they do not receive any of the benefits or services in Section 2, approval is contingent upon income (300% of the Federal Poverty Level) as seen in the chart in Sections 3 and 4.  In Section 3 you must fill out the Family Unit Size, the Yearly Income, and the Monthly Income. In Section 4 you must fill out the names of everyone in the household, starting with yourself, their relationship to you, their DOBs, their income source, their monthly income, and how often received. If they do not have an income, just leave it blank. The number of names must match the number you put in Section 3 for Family Unit Size. You must upload proof of income (such as paystubs) for the last 30 days for everyone in your household and it must match the income reported in Section 4.
